In-Home Health Assessments: Info for Health Plan Members

Your health plan has you covered. Focus Care’s in-home health assessments (IHAs) give your health plan a comprehensive overview of your health, to make sure you can access the care you need for chronic or emerging conditions. Our licensed nurse practitioners visit you in the comfort of your home. They ask questions, observe, listen, and do (or prescribe) diagnostic tests to close care gaps and help you develop a plan to optimize your healthcare. This is a complimentary service designed to benefit you.

How In-Home Assessments Benefit You

Focus Care’s in-home health assessments (IHAs) help your health plan gain insights into your daily life, environment, restrictions, and needs. Our certified health and wellness appraisal tool guides nurse practitioners through each visit to ensure thorough reporting, without any gaps. This proven approach helps us identify health risks early, personalize care plans, and provide guidance to support better health outcomes.

FAQs About IHAs

A Personal Health Visit provides one-on-one time with a Licensed Nurse Practitioner. The Licensed Nurse Practitioner will perform an exam, review health history, and answer any health-related questions.

 If you are selected to participate in this program, you will be contacted by a representative
from Focus Care.

If you are a member of a health plan working with Focus Care, we will be contacting you at the beginning of the year, and we will continue to conduct outreach.

You will be contacted at the phone number and address listed on your account to schedule a visit. You will receive a letter in the mail with instructions and a phone call. Calls occur Monday through Friday between the hours of 8:30 a.m. to 9:00 p.m. EST.

This visit can be in person, or it can be a Virtual Health Visit in the comfort of your home at a time that is convenient for you. There is no cost for the In-Home Health Review.

The In-Home Assessment Program allows you to speak with a Licensed Nurse Practitioner and ask health-related questions in the privacy of your home. The Personal Health Visit does not affect benefits and there is no extra cost.

Please note that participation in the program is not mandatory, nor is it a substitute for the care you receive from your physician. This visit does not replace any previously scheduled doctor’s appointments.
