In this 30-minute session we will provide tips and insights on:

  • Helping residents explore options for community living
  • Risk mitigation planning and identifying risk factors
  • Assisting social workers and other facility staff throughout the process
  • Helping patients understand what services and support they need
  • How life skills training sets residents up for success
