Why refer residents to this comprehensive transition services program?
We understand your pain points in working with a population that is capable of living independently with some support, and the need to make more beds available for those who need them.
In this 30-minute session we will provide tips and insights on:
- Identifying potential independent living candidates among residents
- Helping residents explore options for community living
- Risk mitigation planning and identifying risk factors
- Assisting social workers and other facility staff throughout the process
- Helping patients understand what services and support they need
- How life skills training sets residents up for success
